Case Management

Through Case Management, the needs of a person are assessed and a plan is formulated to meet those needs. Case Managers also make regular checks to ensure that the client's needs are being met.

The needs and problems of the most vulnerable clients are addressed through case management professionals. Case Management is provided to the clients receiving two or more in-home services. These clients are more at-risk for institutionalization and a more comprehensive assessment is needed.

 

Activities include assessing needs, developing care plans, approving services, arranging services, coordinating to provide services among service providers, reassessment and follow up as needed.
